Referral Information for Physicians & Schools


Supporting collaborative, evidence-based functional vision rehabilitation.

Our Vision Rehabilitation & Occupational Therapy Program welcomes referrals from physicians, optometrists, neurologists, rehabilitation providers, case managers, teachers, and school-based support teams. We specialize in functional vision challenges related to concussion, neurological conditions, and developmental visual processing needs.


This page outlines who to refer, what documentation we need, our evaluation process, and how we communicate progress.

Who Should Be Referred

We accept pediatric, adolescent, and adult patients with any of the following:


Neuro-Related Conditions

  • Concussion / mTBI
  • Stroke
  • Brain injury
  • Neurodegenerative or neurological disorders
  • Visual–vestibular dysfunction
  • Visual field loss


Functional Vision Issues

  • Eye tracking or teaming difficulties
  • Binocular vision dysfunction
  • Convergence or accommodative disorders
  • Oculomotor dysfunction
  • Light sensitivity / photophobia
  • Visual processing or perceptual skill deficits
  • Reading difficulties with suspected visual components


School-Based Concerns

Students demonstrating:

  • Reduced reading fluency or stamina
  • Letter/number reversals
  • Copying difficulty
  • Poor attention or visual memory
  • Challenges with worksheets, handwriting, or scanning
  • Frequent headaches or visual fatigue
  • Difficulty transitioning between board → desk → screen

  • Functional visual history
  • Oculomotor performance
  • Eye teaming & convergence efficiency
  • Accommodation / focusing
  • Visual–vestibular integration
  • Perceptual & processing skills
  • Postural and balance responses
  • Light sensitivity
  • Visual field screening
  • Impact on school, work, and ADLs


Submit Referral Form

We accept:

  • Standard physician referral forms
  • School support team referrals
  • Neurology / rehab hospital discharge plans
  • Optometry referral forms

2. Include Relevant Documentation

When available, please attach:

  • Optometric exam findings
  • Neurology or concussion clinic notes
  • PT/OT/SLP notes
  • IEP/504 plans
  • Reading specialist or school psychologist reports
  • Imaging results (if applicable)
  • Medication list
  • Parent/teacher observations


3. Provide Contact Information

Include contact details for:

  • The referring provider
  • A care coordinator or school contact
  • Parent/guardian (for students)


Communication After Referral

We maintain open, timely communication with referral partners.


After the Initial Evaluation, We Provide:

  • Summary of findings
  • Functional challenges identified
  • Initial treatment plan and goals
  • Recommended therapy frequency
  • Projected duration of care


During Treatment, We Provide:

  • Progress updates (as needed or requested)
  • Notes for school accommodations
  • Return-to-learn / return-to-work guidance
  • Adjusted treatment recommendations


At Discharge, We Provide:

  • Final progress summary
  • Recommendations for ongoing support
  • Home or classroom strategies
  • Follow-up care schedule (if needed)
